Medical Release Form

It is a requirement that a Medical Release form be on hand with Team Managers or designated coach during games. We have to do to that as a condition of our agreement with Little League Baseball.

This year we are trying our best to find new ways to keep a record available at all times. Our worse fear would be a sheet being lost or unaccountable during an emergency and a parent not on site. With the new website, you have the ability to place a copy of the Medical Release form into your child's account. I am attaching a copy of the Medical Release form that you can fill in the blanks and save to your computer. I am also providing you a simple presentation with directions on how to upload a copy of the form into your account. I have to emphasize this point again. You shall provide your Team Manager a copy of this form before opening day or your child can't play until the manager can advise the Safety Officer they have all youth medical forms.
